Your first visit to Family Network Chiropractic will consist of a little paperwork, and an exploratory consultation where we’ll:
- get a look at your overall health picture
- make a few minor contacts (early entrainment step)
- and explain what it is that we offer as Network Spinal Analysis chiropractors

There are two forms that need to be filled out before your initial in-person evaluation can take place.

Network Spinal Analysis (NSA) Consent Form
Your agreement to receive Network Spinal Analysis (NSA) Care and descriptions of what that means.
Health, Wellness and Quality of Life Questionnaire
Rate your physical and mental/emotional states, stress levels, ability to experience joy in your life and the overall quality of your life.
Open Room Consent & Authorization
Give your consent and authorization to be treated in the open room with other patients. (NSA works best in group settings.)
Health Insurance Portability & Accountability Act (HIPAA) Authorization
Clarifies how your medical information is used by us and authorizes us to do so.
The Rest Of Your First Visit:
Any preexisting physical or mental conditions will need to be known, as well as any dietary issues and eating habits. Any current stressors that may be negatively impacting your life will need to be shared with us too.
And we’ll give you plenty of opportunity during this first visit to share what it is that you hope to gain from us.
This initial process keeps you safe and makes sure we have all we need to provide stellar treatment moving forward.
It also allows us to respond to any concerns or questions you may have, enabling you to feel calmer and more relaxed about it all before we begin any major care.
Actual entrainments will begin on your next visit.
